Manager or Coach Application Form
IMPORTANT: Prior to submitting your application, please read the Manager/Coach Commitment Statement located HERE. No application will be considered unless this commitment has been agreed to.
In completing this application I am certifying that I am not a user of drugs, do not drink alcoholic beverages in excess, and have not been convicted of a felony. Further, I state that I am not, at this time, or within the past three years, been under psychiatric care, the nature of which would effect my relationship with Little League children.
Any Manager and/or Coach that will have direct and sole supervision over players must have fingerprint clearance prior to the start of the season. The league will pay the fingerprinting fee for one manager and one official coach per team.
